Fareham is a thriving market town of over 30 square miles located between Portsmouth and Southampton. Its population of approximately 118,000 people are served by ten local practices.

As part of the Better Local Care initiative, Fareham GP's passionate about improving patient services are helping to bring together all ten practices to work collaboratively with other health and social services, our local Clinical Commissioning Group and voluntary organisations to improve health care. By looking at the services that patients tell us don’t work or need major improvement, we can support real change. By directing more services into the community and away from harder to reach hospital locations we can make it easier for patients to access the care they need.

Our current focus for change:

  • Frailty: To improve medical care for care home residents, reduce unnecessary emergency calls and admissions, improve the skills of nursing or care home staff
  • Feverish Children: Improve confidence of parents when faced with fever in children, ensure health professionals provide consistent advice specific to the under 5s, reduce unnecessary hospital admissions and the stress this causes to the family, provide easier access to GP appointments
  • Mental Health: reviewing the current medical pathways with a view to improving patient access to appropriate, timely care and support
