What we are doing

In our area we are jointly working with our six General Practices, local communities, organisations involved in delivery of local care and Social Care Services on a number of Better Local Care projects. 

Extended Primary Care 

Our prime focus is on improving care of elderly patients with complex health needs. Our local General Practices work closely together with Southern Health, Hampshire Adult Social Services and local patient groups to remodel the services so that we can better respond to the changing needs and to ensure our approach is more coordinated.

Our General Practices and Integrated Care Teams will be sharing their patient records to enable more efficient way of working together and to help inform solutions for Extended Primary Care. 

Improved Access to Urgent Primary Care 

We are working together to provide a better service for urgent and on the day appointments for our patients. Your feedback and views are valuable to us and are essential to helping us shape how we our work so that we can best meet your needs.

eConsult - A tool to consult with our GP Surgery from

The six General Practices are moving towards introducing eConsult, a service that enables you to receive medical advice without having to go into the practice. More information on this service to follow.

De-layering Specialist Support

We are currently working together with our Health Visiting, Respiratory, Diabetes and Italk colleagues to reduce steps to specialist support services by exploring:

  • option of joint appointments with GPs
  • placement of specialist services in your practice
  • promotion of patient signposting to specialist services
